Website Privacy Policy
J&M Community Services LLC
Effective date: 10/08/2026
1. About this policy
J&M Community Services LLC (“J&M,” “we,” “us,” or “our”) respects your privacy. This policy explains how we collect, use, and share personal information in connection with https://www.jmcommunityservices.com/ (the “Website”) and general inquiries about our services.
This policy covers Website visitors and people contacting us for general information. Participant records, enrollment documents, service plans, billing records, and other confidential service information are subject to applicable law and the privacy and confidentiality notices governing those services. This Website policy does not replace a HIPAA Notice of Privacy Practices where one is required.
2. Information you provide
When you contact us by email, telephone, mail, or a Website inquiry feature, we may receive your name, email address, telephone number, preferred contact method, and the contents of your message. If you identify your relationship to a prospective participant, family member, or waiver program provider, we may also receive that information.
You choose what to provide. We use inquiry information to respond to questions about availability, scheduling, enrollment, Medicaid waiver services, and private-pay options. An initial inquiry does not enroll anyone in services.
Please share another person’s information only when you have appropriate authority. We may verify a representative’s authority before discussing confidential information.
3. Technical information and cookies
Our Website hosting and analytics services may automatically collect technical information such as an IP address, browser and device type, pages visited, referring website, visit times, and information about how the Website is used. This information helps operate the Website, identify errors, protect against misuse, and understand visitor activity.
The Website may use cookies and similar technologies for essential functions and basic analytics. Cookies are small files stored on your device. You can manage or delete cookies through your browser settings and use any cookie controls displayed on the Website. Blocking certain cookies may affect Website functions. Where required by applicable law, optional cookies will be subject to the required consent or opt-out controls.
Browser privacy settings and signals may affect tracking, depending on the browser and Website configuration. We honor legally required opt-out signals where applicable. This policy does not promise that every browser “Do Not Track” setting disables all cookies or technical logging.
4. How we use information
We use information to respond to inquiries, communicate about requested services, arrange tours or follow-up discussions, maintain necessary correspondence, operate and improve the Website, protect information and systems, and meet applicable legal obligations.
Providing contact information for an inquiry permits us to respond to that inquiry; it does not automatically subscribe you to a mailing list or authorize unrelated marketing. We do not use Website visitor information for targeted advertising or advertising-based profiling. We do not sell personal information collected through the Website.
5. How we share information
We may share information with service providers supporting Website hosting, basic analytics, email, communications, and technical support, as needed for their work. We limit sharing to the relevant purpose and require appropriate protections where required by law.
We may disclose information when required by law, a valid legal process, or a lawful regulatory request; to meet mandatory reporting obligations; or as legally permitted to address fraud, security threats, or a serious safety concern. We may also share information at your direction or with the authorization required for the particular disclosure.
A Website inquiry does not authorize unrestricted sharing with a waiver program provider, family member, or other organization. Any sharing of confidential participant information must follow applicable law and relevant authorization requirements. We do not provide participant information to advertisers through the Website.
6. Sensitive information and participant privacy
The Website and ordinary email are intended for general inquiries. Do not submit Social Security numbers, Medicaid identification numbers, detailed medical records, payment card or bank details, or other sensitive records through a general Website form or ordinary email. Contact us first to arrange an appropriate way to share records.
Information about disabilities, health conditions, and services may receive additional protections under applicable law. If we receive such information, applicable confidentiality obligations continue to apply. Sending sensitive information does not authorize its public use or remove those protections.
Website visits and inquiries do not authorize us to publish a participant’s image, testimonial, or identifying information. We obtain any required permission separately before using those materials.
7. Retention and security
We retain inquiry and Website information for as long as reasonably needed for the purpose for which it was collected, including responding to requests, maintaining necessary business records, addressing disputes, and meeting legal obligations. Retention depends on the type of information, its purpose, applicable requirements, and service-provider settings. Participant records follow the requirements and retention policies applicable to those records.
We use reasonable administrative, technical, and physical safeguards appropriate to the information we handle. No website, email system, or electronic storage method can guarantee complete security. If notification of a security incident is required by law, we will provide the required notification.
8. Your choices and privacy requests
You may choose not to provide information, although we may then be unable to respond to a particular request. You may ask us to update your inquiry contact details, use a preferred communication method, or stop optional follow-up communications. You can also request access to, correction of, or deletion of personal information associated with your Website inquiry.
Privacy rights depend on applicable law and the information involved. Where applicable law grants them, rights may include confirmation of processing, access, correction, deletion, a portable copy, and opting out of sale, targeted advertising, or certain profiling. Statutory exemptions and limits may apply. These rights do not require deletion of records we must retain or override the separate rules governing participant records.
Submit a request to info@jmcommunityservices.com with the subject “Privacy Request,” or call 806-731-2486. Describe your request and provide enough information for us to locate the relevant records. Do not attach sensitive identification documents unless we arrange an appropriate method. We may reasonably verify identity or a representative’s authority before acting.
We will respond within the time required by applicable law. If we deny a request, you may ask us to reconsider by contacting the same email address with the subject “Privacy Appeal.” Where a statutory appeal right applies, we will follow its required process and deadlines. You may contact the Texas Attorney General or another appropriate authority about a privacy concern; contacting us first is not required.
9. Children and representatives
Our Website describes services for adults with disabilities and is intended for adults, families, authorized representatives, and service providers. We do not knowingly collect personal information online from children under 13. If you believe a child under 13 has provided personal information through the Website, contact us so we can take appropriate action.
We respect adult participants’ privacy and decision-making rights. A family relationship alone does not establish authority to access records or act on another adult’s behalf.
10. External websites and services
Links to external websites, social media pages, or third-party services lead to services with their own privacy practices. If you follow a link or interact with a third-party service, review that provider’s privacy policy. Its independent practices are outside our control. Information collected by vendors on our behalf remains subject to our applicable responsibilities.
11. Changes to this policy
We may revise this policy when Website features, information practices, or legal requirements change. We will post the revised policy and update its effective date. Where applicable law requires notice or consent for a material change, we will provide that notice or obtain that consent. Posting a revision does not itself authorize uses requiring separate consent.
